Emergency Response Records Request WITH Medical Information
The Authorization for Use & Disclosure of Protected Health Information and Records form must be fully executed with this request and submitted by U.S. mail, in person, or secure email before records with medical information are searched for and provided. Please include a check for $5.00 for each 911 audio CD.
